Emergency Form

It is essential that as parents you give your new nanny or caregiver important emergency information. They must be able to reach you at all times and they must know important numbers for relatives and neighbors in case of an emergency situation. They must also know doctor's names, phone numbers and addresses in case there is an accident and the child(ren) need medical care. NannyCare.com recommends filling out an emergency form so that your nanny/caregiver has access to this information at all times. Make sure you also fill out a "Consent for Medical Care" form just in case your child has a serious accident and you cannot be reached, this way your child can get the care they need and the nanny will have all medical information on hand.
